TIP OF THE WEEK:
What is the Delirium Scale?
The Delirium Score can be found in the Scales section of the Assessment Summary. It is calculated by adding the scores for the items in Section D3 (Cognition) as follows. If 3a, 3b or 3c = 2, then one point is added to the Delirium Score. If Item D4 = 1, a further point is added. Maximum score is 4. The higher the score, the more symptoms of delirium are present, and the greater the likelihood that the person has delirium.
